This is zeroed from the center point. I did a couple tests on scrap before doing this on Table Saw extension. I am bit leery of sharing a file as I am not so good at all this stuff. Measure twice and all that. This is my lift, your plate dimensions and thickness may be different. JessEm Mast-R-Lift II - Lee Valley Tools

My used Powermatic table saw came with a router in the extension. They did a horrible job and it bugged the hell out of me. I did the same thing as you but I didn’t have a CNC at the time. IIRC the corners were a .75" radius so I used a hole saw to make each ‘corner’ and router to remove the material to the correct depth. A CNC would have been so much easier.

I was recently given a cast iron router table that wasn’t set up for a router plate. I keep looking at it but have yet to try and cut the table to mount a plate. Too bad I could do it on the 1F.
